Forward Championship Roster of qualifying team(s) to State JO Commissioner within 48 hours of complete of tournament.

Forward copy of completed form with your payment of team assessment ($25 per team) to the State Office at the address above within 7 days of completion of tournament. Make check payable to ASA of PA.
- STATE CHAMPIONSHIPS:
- Forward ALL Championship Rosters to State JO Commissioner within 48 hours of completion of tournament.

Submit team photo of Championship team via email to State JO Commissioner within 48 hours of completion of tournament.

Forward copy of completed form with Tournament Financial Report and proceeds to State Office within 7 days of completion of tournament. (Check made payable to ASA of PA)
