To find your District Commissioner, click on the "STAFF" tab at the top of the page and go to "District Map". Once you have printed a copy of this form, click on the "SUBMIT TO STATE OFFICE" button below. A copy of your information will be forwarded to the ASA of PA State Office.

Please complete ALL requested information and print a copy of the form to submit to your District Commissioner with your Sanction Fee check made payable to "ASA of PA".

All tournaments must be sanctioned with ASA of PA in order to utilize ASA umpires.

As a condition of obtaining this sanction, the above organization agrees to abide and be governed by the rules and regulations of non-championship tournaments as set forth in the current ASA Code.

NOTE: All Junior Olympic (youth) Eastern National Qualifiers require pre-approval by the State JO Commissioner. Eastern National Qualifiers are also subject to a $25 per team assessment which must be submitted within 7 days of completion of tournament along with the Qualifier Report listing the participating teams in the order of finish.
